Claim Process Simplified

Simple steps to help you file a claim under your insurance policy.

  • 1 Upon discharge, pay all hospital bills and collect all original documents of treatment undergone and expenses incurred
  • 2 Download the GCI health claim form, fill it and along with it
  • 3 Submit original documents to the nearest Generali Central branch
  • 4 We will settle the claim in subject to policy terms and conditions, with a promised Turn Around Time of 10 days, from the last document received date

Document Checklist

To process your reimbursement claim, please ensure the following documents are submitted within 30 days of hospital discharge, and the claim is intimated to GCI Call Centre within 48 hours of admission:

Hospitalization Claim

  • Duly signed GCI Claim Form (by claimant or family member)
  • NEFT details (Cancel cheque/passbook copy/bank statement) of employee or Proposer.
  • Photocopy of GCI Health ID Card
  • Original Hospital Discharge Card
  • Original itemized hospital bill (with seal & signature)
  • Original paid receipt (with revenue stamp, seal & signature)
  • First Prescription / Consultation Letter (Photocopy)
  • Lab & Diagnostic Reports (X-Ray, ECG, MRI, Blood test etc.)
  • Medicine bills (if bought separately) + Doctor’s prescription
  • Diagnostic/Radiology bills (if paid separately) + Doctor’s advice
  • IOL Sticker (for Cataract cases only)
  • Accident Declaration + FIR & MLC (if applicable)
  • Hospital Registration Certificate (for non-network hospitals)
  • PAN & photo ID proof of Proposer (Retail policies only)

Pre-post Hospitalization Claim

  • Signed GCI Claim Form (by claimant or family member)
  • NEFT details (Cancel cheque/passbook copy/bank statement) of employee or Proposer.
  • Xerox of Health ID Card
  • Medicines: Doctor’s prescription and corresponding chemist bill
  • Doctor’s Consultation Charges: Prescription, doctor’s bill, and payment receipt
  • Diagnostic Tests: Prescription for tests, test reports, and diagnostic centre bill with receipt.

Temporary Total Disability (TTD)

  • Claim form with attending physician’s statement
  • Initial treatment prescriptions and medical records
  • Fitness certificate/ leave records with disability duration
  • Self-declaration of injury details
  • Diagnostic reports (X-rays, etc.)
  • FIR/MLC (if applicable)
  • ID/address proof
  • NEFT form with bank details and cancelled cheque

Permanent Partial Disability (PPD)

  • Claim form with attending physician’s statement
  • Disability certificate (type & percentage)
  • Diagnostic reports
  • FIR/MLC (if available)
  • ID/address proof
  • NEFT form with bank details and cancelled cheque

Permanent Total Disability (PTD)

  • Claim form with attending physician’s statement
  • Disability certificate from Civil Surgeon
  • Diagnostic reports
  • FIR/MLC (if available)
  • ID/address proof
  • NEFT form with bank details and cancelled cheque

Accidental Death

  • Claim form signed by nominee
  • Death certificate
  • FIR/Panchnama, Post-Mortem & Viscera reports
  • Witness statement (if any)
  • Burial certificate (if applicable)
  • Nominee’s ID/address proof
  • Legal heir certificate with affidavit & indemnity bond
  • NEFT form with nominee’s bank details and cancelled cheque

Accidental Hospitalization

  • Claim form with physician’s statement
  • Original discharge summary and hospital bills
  • Pharmacy bills with prescriptions
  • Diagnostic reports and investigation bills
  • FIR/MLC (if available)
  • ID/address proof
  • NEFT form with insured’s bank details and cancelled cheque

  • cp_icon_damage 2 The Network Hospital will verify your identity and submit duly filled pre-authorization form with Generali Central Insurance. We will review the pre-authorization request and respond to the hospital, within promised Turn Around Time of 1 hour
  • cp_icon_qualify_bl 3 You get admitted without any deposit and get cashless treatment
  • cp-submit-documents 4 Once you are discharged, the hospital will send your claim documents to us. We will review and reply to the hospital, within promised Turn Around Time of 3 hours

To apply for cashless treatment, please visit any of our network hospitals. At the time of admission, ensure the following documents are available:

  • Duly filled Pre-authorization form
  • Insurance Card
  • Employer Card (Corporate employees)
  • Doctors’ consultation paper for hospitalization and medical documents
  • Investigation Test Reports (if applicable)
  • KYC document (if applicable)
  • Photo ID proof
